Who can get it?
Health support is available for people over the age of 18 who
have a learning disability. The Community Nurse Team also
works with young people who may be moving from Children's to Adult
Services to get a 'Transitions Health Plan'.
What
is a learning disability?
As a learning disability is usually present from childhood, this
would normally be people who are already receiving other services
from us. If not, we would need to carry out an assessment.

How much does it cost?
There is no charge for health assessments and services.
The cost is covered for you by the NHS.
How to get in touch
If you already receive services from us then please speak to
your Social Worker. If not, the Adult Social Care Team is the first
point of contact for all referrals and general enquiries. You can
contact them from 8.45am to 5pm, Monday to Friday on (01254)
587547.
When you contact us we'll discuss your situation over the phone,
check that we've got all our facts straight and then either follow
this up with a visit to your home or arrange for you to come to see
us.
